In order to make an extract from the TAXI license, we must have a manually completed car lending agreement. The contract must include the data of all owners of the car.
Please print the contract, fill in the details, all owners of the vehicle must sign on the last page. The original of this agreement must be delivered to our office in paper form because it is necessary for presentation to the office.
Send the original contract along with the other documents for the cab license to our mailing address, i.e. ul. Traugutta 25 lok. 917, 90-113 Lodz, or deliver to any of our offices.
